Parents and Student Athletes,
While we are excited to see the return of athletic activities in our community, the health and safety of our student athletes is our upmost priority. Please read through the attached Re-Entry Plan carefully. We want to thank you in advance for your patience and understanding as we begin the process of returning to the athletic fields.
There are a few items to take special note of:
The IHSAA is allowing physicals from last year (2019-20) to be valid for this year (2020-21).
In some instances, a student athlete may be required to get a new physical. If this is the case, that physical will be due to the athletic office by the first official practice date of the sport.
In order for your 2019-20 physical to be valid for the 2020-21 school year, student athletes must fill out and return (to their coach) a copy of the IHSAA Sports Health Questionnaire and the Consent and Release Certificate. These forms must be signed and returned no later than July 10.
When filling out the IHSAA Sports Health Questionnaire, keep in mind that the 7 questions asked are, “Since the last pre-participation physical examination.” For most of our athletes, this would be from last year’s physical.
The online paperwork from the 2019-20 school year is NOT valid for the 2020-21 school year. Students and parents will need to go to their Final Forms Account and sign their forms digitally. Forms have been added to this within the last 24 hours. Please review before July 6.
All Final Forms must be completed before student-athletes can attend summer work outs or practices on July 6th.
